U.S.A.  ______  [illegible].  Clay is not yet able to travel and I herewith report [strike through report] send the certificate of His attending physician  Clay is very anxious to return as soon as able.  The physicians certificate you will observe has been sent to Boston _____ [illegible] and returned -
