�19 To all whom it may concern Know Ye, That Dudly Chase a private of Capt H. T. Buckmans Co. K 5th Regiment of Maine Vols who was Enrolled on the 24th day of June 1861 to serve three years is hereby Discharged from the service of the U.S. this first day of October 1861 at Camp Lyon Va by reason of Ang Pectoris - No objection to his being reenlisted is known to exist [whole phrase lined out] Said Dudly Chase was born in Waterboro in the State of Maine is 38 years of Age 5 feet 7 inches high Sandy Complexion blue Eyes Light hair and by occupation when Enrolled a Laborer Given at Alexandria Va this third day of October 1861 N.J. Jackson Col, Com, 5th Me Regt [t underlined]

The above named Dudly Chase was Discharged by reason of a Rheumatic Complaint which rendered him unfit for the service - He has tried to Enlist again but was rejected - E.B. Lovejoy
