On the twentyfirst day of June in the year of our Lord Eighteen Hundred and Twenty Three personally appeared in Open Court Viz in the Supreme Judicial Court, holden at Bangor within and for said County, the same being a Court of record which proceeds according to the [?] of the Common law, with a Jurisdiction unlimited in pursuit of amount Ezekiel Chase now resident at a place called Seboeis in said county, but late of Sebec in the same county, aged sixty two years, being first duly sworn according to law doth on his oath make the following declaration in order to obtain the Provision made by the  Acts of 18th March A.D. 1818 - first of May A.D. 1820 and first of March 1823 that he the said Ezekiel Chase enlisted in the State of Massachusetts in the year 1778 for the term of three years and served as a private soldier in the company commanded by Captain Benjamin Buxton in Colonel Henry Thurmans[?] Regiment in the Rhode Island Line, and in the company commanded by Captain Hastings in Colonel Henry Jacksons Regiment in the Massachusetts Line and  Captain William Scott's Light Infantry company in the Regiment commanded by Colonel Henry Ogden

And I do solemnly swear that I was a resident citizen of the United States on the eighteenth day of March A.D. 1818 and that I have nor since that time by gift, sale, or in any manner disposed of my property or any part thereof, with intent [stricken] thereby to diminish it as to bring myself within the provisions of an Act of Congress entitled "An Act to provide for certain persons engaged in the land and naval service of the United States in the Revolutionary war" passed on the 18th day of March A.D. 1818, and I have not nor has any person for me, any property or securities, contracts or debts, due to me; nor have I any income other than what is contained in the schedule hereto annexed and by me subscribed.
