I have this day, carefully examined Thomas B. Chalmers Co. G. Capt Sergeant 2nd Regt M.V.M.--and find that he is suffering from an affection of the neck (scrofula) and also and affection of the right testicle and in consequence thereof in my opinion is unfit for military duty and that the affection is permanent.
