On this Eighteenth day of June A.D. 1830 personally appeared in open court to wit in the Supreme Judicial Court now holden at Bangor witness before the County of Penobscot the same being a Court of [?] proceeding according to the Course of the common law with a jurisdiction unlimited in payment of amount John Blake resident in said county aged seventy seven years who being  first duly sworn according to law. Doth on his oath make the  following declaration in order to obtain the Provision made by the acts of Congress of the 18th of March 1818 and the 1st of May 1820 that he has said Blake Enlisted for the term of Eight years of the 19th day of April 1775 in the line of the State of Massachusetts in the Company Commanded by Capu- Oliver Pom in the Regiment Commanded by Col. Joseph Reed & that he continued to serve in said corps until the expiration of said eight months - and that in January 1776 he enlisted in the same company commanded by the same Captain and belonging to the same Regiment Commanded by said Reed for the term of one year as sergeant & continued to serve in said Corps until January 1777. And that in Jany 1777 he was appointed a commission as Ensign in the Company Commanded by Capt Nathan Goodale in Col Rufus Putnams regiment in the Massachusetts line - & that he continued to serve in said Goodales Company until the 24th of October 1780 when he was honorably discharged being at that time a Lieut. in said Putnam's Regiment - at the place when Major Andre was executed as a spy - [text crossed out]
