BACKGROUND Immunoglobulin (Ig)G4-related disease is classified as an immune-mediated disease. The etiology of this condition has not been explained to date. Manifestations of the disease are diverse, and simultaneous involvement of multiple organs is not unusual. CASE REPORT We report the case of a patient referred to us after multiple unsuccessful paranasal sinus operations who presented with enophthalmos and a resultant migratory keratitis with a suspected diagnosis of silent sinus syndrome. Preservation of the orbit was no longer feasible. After five years without a definitive diagnosis, we ascertained that this was a case of IgG4-related disease. DISCUSSION IgG4-related disease represents an important element in the differential diagnosis of chronic advanced diseases of the orbit and paranasal sinuses. The diagnosis should be considered in the case of unclear disease presentations. Typical histological findings include a storiform pattern of fibrosis, vasculopathy, and tissue infiltration by IgG4 plasma cells.PURPOSE Many stroke patients with large vessel occlusion present with a low National Institutes of Health Stroke Scale (NIHSS). There is currently no level 1A recommendation for endovascular treatment (EVT) for this patient subgroup. From a physician's standpoint, the deficits might only be slight, but they are often devastating from a patient perspective. Furthermore, early neurologic deterioration is common. The purpose of this study was to explore endovascular treatment attitudes of physicians in acute ischemic stroke patients presenting with low admission NIHSS. METHODS In an international cross-sectional survey among stroke physicians, participants were presented the scenario of a 76-year-old stroke patient with an admission NIHSS of 2. Survey participants were then asked how they would treat the patient (A) given their current local resources, and (B) under assumed ideal conditions, i.e., without external (monetary or infrastructural) constraints. Overall, country-specific and specialty-specific decision rates were calculated and clustered multivariable logistic regression performed to provide adjusted measures of effect size. RESULTS Two hundred seventy-five participants (150 neurologists, 84 interventional neuroradiologists, 30 neurosurgeons, 11 affiliated to other specialties) from 33 countries provided their treatment approach to this case scenario. Most physicians favored an endovascular treatment approach, either combined with intravenous alteplase (55.3% under assumed ideal and 52.0% under current working conditions) or as single treatment (11.3% under assumed ideal and 8.4% under current conditions). CONCLUSION Despite the limited evidence for endovascular therapy in acute stroke patients with low NIHSS, most physicians in this survey decided to proceed with endovascular therapy. A randomized controlled trial seems warranted.Previous studies indicated a positive effect of vitamin K2 (VK2) supplementation on bone turnover biomarkers and bone mineral density (BMD), but the doses varied, and few studies have focused on the difference between VK2 supplementation alone and in combination with calcium and vitamin D3. Immunology inhibitor The aim of this study was to explore a low and effective dose of VK2 for improving BMD, and to examine whether the co-supplementation of VK2, calcium and vitamin D3 would bring greater effects. In this trial, a total of 311 community-dwelling men and postmenopausal women aged 50 and 75 years were randomly assigned to four groups, receiving placebo, 50 µg/day, 90 µg/day or co-supplementation with calcium (500 mg/day) and vitamin D3 (10 µg/day) for 1 year. At the endpoint, the bone loss of femoral neck was significantly lower in postmenopausal women in the two 90 µg groups (treatment × time, p = 0.006) compared with placebo, but no effects in men. Serum biomarkers cOC/ucOC ratio increased in the intervention groups (treatment × time, p  less then  0.001). VK2 supplementation in dose of 90 µg/day performed a significant effect on reducing bone loss in postmenopausal women, but in combination with calcium and vitamin D3 brought no additional effects.Trial registration This trial was registered at http//www.chictr.org.cn as chiCTR1800019240.Twenty-eight transects in six urban rivers located in the Qianhai area of Baoan District, Shenzhen, China, were selected to study the contamination of heavy metals in its sediment, reveal the spatial distribution of heavy metals in these rivers, and assess the ecological risk of heavy metals in the urban river sediment. The result showed that the concentration of heavy metals displayed considerable variation in the study area. The results showed that southern airport drainage river had exposure to high concentrations of Cu, Nanchang Chung exposure to high concentrations of Zn, Cr, and Pb, Gongle Chung exposure to high concentrations of Ni, and Xixiang River exposure to high concentrations of Hg. The results of correlation analysis and cluster analysis indicated that Cu had significant correlations with Zn and Cr respectively (P  less then  0.05). There was significant correlation, and the same cluster existed among Zn, Cr, and Pb (P  less then  0.01), whereas Hg was not significantly correlated with other metals. The potential risk of the six rivers could be sorted as Gushu Chung  less then  Gongle Chung  less then  Tiegang reservoir flood discharge river  less then  Xixiang River (a), Nanchang Chung  less then  and Southern Airport drainage river.We have previously proposed a model of motor lateralization that attributes specialization for predictive control of intersegmental coordination to the dominant hemisphere/limb system, and control of limb impedance to the non-dominant system. This hypothesis was developed based on visually targeted discrete reaching movement made predominantly with the shoulder and elbow joints. The purpose of this experiment was to determine whether dominant arm advantages for multi-degree of freedom coordination also occur during continuous distal movements of the wrist that do not involve visual guidance. In other words, are the advantages of the dominant arm restricted to controlling intersegmental coordination during discrete visually targeted reaching movements, or are they more generally related to coordination of multiple degrees of freedom at other joints, regardless of whether the movements are discrete or invoke visual guidance? Eight right-handed participants were instructed to perform alternating wrist ulnar/radial deviation movements at two instructed speeds, slow and fast, with the dominant or the non-dominant arm, and were instructed not to rotate the forearm (pronation/supination) or move the wrist up and down (flexion/extension).
