We identify a lack of high-quality evidence of systemic biomarkers in tailoring treatment strategies relative to patient-specific risks, but recognise the potential held within biomarker-based diagnostic studies to improve management of hernia pathogeneses.

In patients with ductal carcinoma in situ (DCIS), the risk of developing contralateral breast cancer (CBC) and the role of adjuvant endocrine therapy (ET) in decreasing CBC risk is not well understood.

A retrospective review was performed on patients with DCIS who underwent a unilateral mastectomy (UM) from 2000 to 2010. CBC incidence, time to CBC and disease free survival were evaluated for women who took ET versus those who did not.

176 patients underwent a UM for DCIS, 72% were ER positive and 16% of patients took ET. At a median of 12 year follow up, 15 patients experienced CBC. Women over 63 years had highest incidence of CBC. CBC risk was not different between those who took ET versus those who did not.

Efforts to improve surgical safety are limited by several factors and no consensus exists regarding the most effective way to improve surgical quality. The use of ISO 9001 quality standards within healthcare is recognized but has not been widely applied for improving surgical outcomes.

A surgical quality committee was created using ISO 90012015 standards. Quality objectives were assessed to understand how any suggested changes will be impacted due to risks and opportunities inherent in the system.

The initial quality focus was on surgical site infections in 5 services. Change in surgical infection ratio from 2018 to 2019 showed significant improvement coronary bypass 1.288 vs. 0.901; Colon 1.359 vs. 0.589; Hysterectomy 2.119 vs. 1.022; Knee 1.391 vs. 0.306; Hip 0 vs. 0.302.

The COVID crisis hit during the interview season for the Complex General Surgical Oncology (CGSO) fellowship. With minimal time to adapt, all programs transitioned to virtual interviews. Here we describe the experience of both program directors (PDs) and candidates with virtual interviews, and provide guidelines for implementation based on the results.

Surveys regarding interview day specifics and perceptions were created for CGSO fellowship PDs and candidates. They were distributed at the conclusion of the season, prior to match.

Thirty (94%) PDs and 64 (79%) candidates responded. Eighty-three% of PDs and 79% of candidates agreed or strongly agreed that they felt comfortable creating a rank list. If given the choice, 60% of PDs and 45% of candidates would choose virtual interviews over in-person interviews. The majority of candidates found PD overviews, fellows only sessions and pre-interview materials helpful.

Our results also confirm key components of a virtual interview day.The fault detection for a class of continuous-time nonlinear networked control systems with medium access constraint is concerned in this paper, where the occurring probability of transition from one sensor to another is allowed to be partially unknown and uncertain. First of all, a Markovian system approach is adopted to describe the access process of sensors, in which only one sensor is allowed to access the communication channel. A robust filter based residual generator is proposed to generate the residual signal such that it can be used to indicate whether the fault has occurred or not. The nonlinear term is approximated by a neural network, and the Lyapunov-Krasovskii functional is introduced to analyze the fault detection system, three sufficient conditions for the stochastic stability of fault detection error system are given, and the fault detection filter gains are calculated via solving some matrix inequalities. In the simulation, a second-order DC motor system is used to validate of the main results, which shows the effectiveness of the fault detector design.A new sliding mode observer design scheme is developed with a novel linear sliding manifold for the exponential stabilization problem of a class of uncertain switched systems in this note. Firstly, the linear sliding manifold is constructed based on a modified observer, through which a new technical approach of the underlying system analysis is exploited to devise a novel exponential stability criteria. Moreover, an associated adaptive switching controller is presented, by which the arrival condition of the designated sliding manifold is fulfilled. Ultimately, illustrative examples are conducted to confirm the feasibility of the proposed scheme.
