I have this day, [ fully] excamined Thomas B. Chalmers Co. G. Capt Surgical 2nd Regt M.V.M.--and find that he is suffering from [ ] affection of the neck (scrofula?] and also and affection pf the right tes -ticle and in consequence thereof in my opinion is unfit for military duty and that the af -fection is permanent.
