Dear Sir, I take the liberty to address a issue

in connection with this a 

desire of obtaining a discharge which the Dr. said I should have. I should have as soon as I get my discharge list as I am unfit for duty and if you will be so kind as to forward my work.

It will oblige me very

much I presume you know who I am I having Trained in your company and worked at your house on Stilwater Ave. I tried to get a furlough so not to leave the service but the Dr said I should not be fit for any more service
