Patients with metastatic breast cancer are usually considered incurable. Recent advances have resulted in significant improvements in survival for patients with metastatic breast cancer. Due to the lack of randomised trials and heterogeneous disease biology, treatment decisions for patients with oligometastatic breast cancer vary widely. Some patients are treated similar to those with widespread disease while others are treated more aggressively. We conducted a review of the evidence for treatment options in oligometastatic breast cancer and consulted ClinicalTrials.gov to explore currently accruing or studies in development aimed at investigating oligometastatic disease in breast cancer. Surgery to the primary tumour in patients with metastatic breast cancer has failed to show any advantage over systemic therapy. However, there may be a benefit in women with controlled systemic disease who are hormone receptor positive with bone-predominant metastasis. Stereotactic radiotherapy has gained increased interest in this setting due to its excellent efficacy and lower rates of associated toxicity. A significant challenge remains in identifying the patient population who would benefit from such an approach, and to do so, we need to understand the distinct biology of oligometastatic breast cancer. Unique miRNA expression and low levels of tumour infiltrating lymphocytes in the immune micro-environment have been described in tumour tissues in patients with oligometastatic breast cancer. There is ongoing research aimed to better characterise these tumours, thus, allowing the selection of patients who would truly benefit from multi-modality treatment in an attempt for long-term survival and cure.Extracellular vesicles (EVs) are cell-derived nanoscale vesicles involved in intracellular communication and the transportation of biomarkers. EVs released by mesenchymal stem cells have been recently reported to play a role in cell-free therapy of many diseases. Selleckchem DT2216 However, the demand for better research tools to replace the tedious conventional methods used to study EVs is getting stronger. EVs' manipulation using alternating current (AC) electrokinetic forces in a microfluidic device has appeared to be a reliable and sensitive diagnosis and trapping technique. Given that different AC electrokinetic forces may contribute to the overall motion of particles and fluids in a microfluidic device, EVs' electrokinetic trapping must be examined considering all dominant forces involved depending on the experimental conditions. In this paper, AC electrokinetic trapping of EVs using an interdigitated electrode arrays is investigated. A 2D numerical simulation incorporating the two significant AC electrokinetic phenomena (Dielectrophoresis and AC electroosmosis) has been performed. Theoretical predictions are then compared with experimental results and allow for a plausible explanation of observations inconsistent with DEP theory. It is demonstrated that the inconsistencies can be attributed to a significant extent to the contribution of the AC electroosmotic effect. Two studies were conducted to determine the effect of the supplementation of sugar, starch or both on feed, nutrient and energy intake and total tract digestibility in four adult female addax (Addax nasomaculatus) and four adult male Reeves's muntjac (Muntiacus reevesi) - representatives of grazing and browsing ruminants, respectively. Studies in both species were conducted according to 4 × 4 Latin Square Design. Animals had free access to meadow hay (addax) or dehydrated chopped lucerne (muntjac), and were fed a restricted amount of a 'basal diet' consisting of (1) wheat bran; (2) wheat bran and sucrose (source of sugar); (3) wheat bran and wheat (source of starch); or (4) wheat bran, sucrose and wheat. The amounts of supplemental sucrose and wheat were set to account for 2% and 15%, respectively, of dry matter (DM) consumed. There was no effect of the ~2% sugar supplementation on DM intake of hay by addax, while the ~10% starch supplementation reduced DM intake of hay by 13% (p less then 0.01); total DM intake (of hay and the basal diet) was not affected neither by sugar nor starch supplementation. When the diet for addax included wheat, this resulted in a greater intake of crude protein by 15%, lower intake of ADF by 9%, and greater crude protein digestibility by 10% (p ≤ 0.05). The ~2% sugar supplementation did not affect intake of lucerne and total DM intake by muntjac, but the ~10% starch supplementation decreased DM intake of lucerne by 25% (p less then 0.01), total DM intake by 7% (p = 0.02) and intake of all nutrients (p ≤ 0.10). In summary, if high intake of roughages by captive ruminants is fundamental for their gastrointestinal functions and health, then starchy feeds supplementation should be limited, as they have an especially negative impact on roughage intake.Exposure to bisphenol A (BPA), triclosan and parabens is widespread but their impact on breast cancer risk remains unclear. This nested case-control study investigated endocrine-disrupting chemicals (EDCs) and breast cancer risk within the Multiethnic Cohort (MEC). We measured prediagnostic urinary BPA, triclosan and parabens in 1032 postmenopausal women with breast cancer (48 African American, 77 Latino, 155 Native Hawaiian, 478 Japanese American and 274 White) and 1030 individually matched controls, using a sensitive and validated liquid chromatography mass spectrometry assay. Conditional logistic regression was used to examine risk with these EDCs with adjustment for creatinine and potential confounders. In all women, breast cancer risk was not associated with BPA (Ptrend = 0.53) and was inversely associated with triclosan (ORT3 vs T1 = 0.83, 95% CI 0.66-1.04, Ptrend = 0.045) and total parabens (ORT3 vs T1 = 0.77, 95% CI 0.62-0.97, Ptrend = 0.03). While risk of hormone receptor positive (HR+) cancer was 20% to 23% lower among women in the upper two tertiles of paraben exposure (Ptrend = 0.02), risk of HR negative (HR-) was reduced 27% but only among those in the upper tertile of exposure. Although risk associations did not differ significantly by ethnicity or by body mass index (BMI), the inverse association with triclosan was observed mainly among overweight/obese women (ORT3 vs T1 = 0.76, 95% CI 0.56-1.02, Ptrend = 0.02). In summary, breast cancer risk in a multiethnic population was unrelated to BPA and was weakly inversely associated with triclosan and paraben exposures. Studies with multiple urine samples collected before breast cancer diagnosis are needed to further investigate these EDCs and breast cancer risk.
